The APCO–IOF Asia Pacific Regional Audit on the epidemiology, costs and burden of osteoporosis 2025 is not just a regional report; it is a systems stress test for global osteoporosis preparedness and ageing. Across 22 countries, structural gaps recur: incomplete fracture surveillance, uneven diagnostic access, misaligned reimbursement, and limited secondary fracture prevention. These are not failures of knowledge but of system design. The lessons are universal: count fractures, define quality, fund post-fracture pathways, develop coherent guidelines, align financing with risk, and track outcomes. Osteoporosis is not a niche specialty concern but a defining test of how health systems respond to demographic ageing.
