Medstracker: a design science approach to medication management in Nigeria

Medication adherence is a significant challenge in Nigeria, exacerbating chronic conditions, increasing healthcare costs, and undermining treatment outcomes. Existing mobile adherence tools often overlook cultural contexts and fail to differentiate reminders based on medication type, limiting their impact in low-resource settings. This study introduces Medstracker, a mobile application that uniquely integrates culturally relevant strategies, medication-type-specific symbolic nudges (e.g. icons for tablets, syrups), and persuasive technology to improve adherence. Guided by the Design Science Research methodology, the study followed an iterative process of problem identification, solution objectives, design, development, and evaluation. Medstracker’s features include dynamic reminders, adherence dashboards, and a user-centred interface for improved usability, supported by a layered Firebase architecture for secure data storage and effective reminder management. Expert reviews with healthcare professionals and interaction design specialists indicated its potential to address barriers such as forgetfulness and complex regimens, whilst offering advantages in cultural relevance and intuitiveness over conventional adherence apps. Recommendations included enhancing security, integrating caregiver support, and conducting real-world usability tests. This research demonstrates how combining user-centred design with culturally adaptive, medication-type-specific nudges can fill a critical gap in digital health solutions, offering a scalable approach for improving health outcomes in Nigeria and similar contexts.
