<p>Posterior cruciate ligament (PCL) injuries are diagnostically and therapeutically demanding. Treatment decisions depend on the injury pattern, the extent of posterior instability and concomitant injuries. Isolated PCL tears with a posterior tibial translation less than 8 mm have good healing potential when managed with consistent bracing and quadriceps-focused rehabilitation. Displaced bony avulsions require surgical refixation. Combined injury patterns involving the posterolateral or posteromedial corner as well as multiligament knee injuries warrant surgical treatment. This article provides a&#xa0;practice-oriented overview of the currently available evidence and formulates an algorithm for making decisions.</p>
