<p>The spreading root sign is a&#xa0;recently described predictor of medial meniscus posterior root tears (MMPRTs) that remains widely underrecognized. This case report highlights the severe consequences of overlooking this sign. A&#xa0;68-year-old female presented with left knee pain. Initial MRI revealed subchondral tibial edema near the medial meniscus posterior root, indicative of the spreading root sign. Conservative treatment was initiated. Two weeks later, follow-up MRI confirmed a&#xa0;complete MMPRT. Despite quick arthroscopic repair, the condition deteriorated, necessitating total knee arthroplasty within 18&#xa0;months. This case underscores the importance of the spreading root sign as an early indicator of an impending MMPRT. Clinicians must be vigilant for this sign to prevent rapid osteoarthritic progression and the subsequent need for joint replacement.</p>
