<p>Arthrofibrosis of the knee predominantly occurs postoperatively or after knee injuries and leads to substantial restrictions in the range of motion and pain, severely impairing the quality of life and function of patients. The disease is characterized by an unusual increase in the periarticular extracellular matrix due to inflammatory dysregulation. It is classified into secondary arthrofibrosis, which arises after surgical interventions or trauma and the rare primary arthrofibrosis without an identifiable cause. Arthroscopic arthrolysis is a&#xa0;minimally invasive and low-risk treatment for this condition. Preoperative diagnostics and clarification of the causes are prerequisites for optimal surgical planning and successful outcomes. A&#xa0;systematic approach to optimal portal selection and the sequence of arthrolysis can help ensure the restoration of joint function through an arthroscopic route, even in cases of severe arthrofibrosis. This article provides insights into the preoperative diagnostics, indications, technical aspects and outcomes of arthroscopic arthrolysis.</p>
