Background <p>Overload reactions are severely limiting and stressful illnesses and in most cases the foot is affected. This overview article presents the most frequent clinical symptoms and explains the background and treatment measures. It serves as a&#xa0;treatment pathway and algorithm in the clinical practice for the treatment of overload reactions of the foot.</p> Methods <p>The discipline-specific literature was systematically evaluated according to clinical presentation, basic principles and treatment concepts and correlated with own experiences and treatment measures.</p> Results <p>Plantar fasciitis, achillodynia, fatigue fractures, shin splint syndrome, overloading of the posterior tibial tendon, Morton′s neuroma and calcaneal apophysitis are the most frequent overload reactions in the foot. The basis of the diagnostics should always be the medical history and carrying out clinical examinations in static and dynamic forms. For imaging, X‑rays of the foot and ankle should be taken in a standing position. In addition, a magnetic resonance imaging (MRI) examination is usually recommended as it provides valuable information about the illness and can also be used to rule out other differential diagnoses. The primary treatment consists of conservative measures. There is increasing evidence for autologous blood treatment with platelet-rich plasma (PRP) for the treatment of plantar fasciitis and achillodynia. After conservative treatment has been exhausted, surgical treatment can sometimes be considered in special cases.</p> Conclusion <p>Overload diseases of the foot are painful and stressful diseases. The primary treatment is conservative. There is increasing evidence for autologous blood treatment with PRP. Nevertheless, patients sometimes go through a&#xa0;long period of suffering.</p>
