Bone marrow edema syndrome is a frequent clinical picture in orthopedics and trauma surgery. The spectrum is broad and ranges from a less painful, self-limiting clinical picture to complex, painful, recurrent or migrating bone marrow edema syndromes up to the threat of transition to osteonecrosis. The diagnosis is made using magnetic resonance imaging (MRI). Knowledge of the etiology is important for the treatment decision. In principle, bone marrow edema syndromes can be divided into three categories: vascular ischemic, mechanically caused and reactive. The transitions are sometimes fluid. The more pronounced a bone marrow edema syndrome is, or in the case of recurrent and multilocular bone marrow edema syndromes, underlying disorders of bone metabolism or chronic diseases must be considered as triggering factors.
