Due to the demographic changes and an aging population, the number of patients affected by osteoarthritis is constantly increasing. In addition, the incidence of degenerative problems, such as knee osteoarthritis is also increasing in younger patients due to a significant increase in the incidence of cartilage, cruciate ligament and meniscus injuries during recreational activities. Knowledge of the underlying pathologies and the treatment options is essential to provide an optimal treatment recommendation. In this context, high tibial osteotomy (HTO) is one of the possible treatment options that should be considered, especially in younger patients with high physical demands and leg axis malalignment. The aim of this review article is to provide a comprehensive presentation of the indications, correct assessment of the leg axis up to planning and surgical treatment. Special attention is given to the latest scientific findings and parameters that have gained in importance in recent years.
