Patellar dislocation with dysplastic trochlea is a common issue, especially among adolescents. Studies indicate an incidence of 23 to 42 primary patellar dislocations per 100,000 patients per year. The patellofemoral joint plays a crucial role in knee function, particularly during everyday activities and athletic endeavors. The treatment of patellar dislocation depends on various factors, including individual morphological risk factors and the clinical presentation of patients. Different risk factors such as the presence of severe trochlear dysplasia have been identified as prognostic factors for recurrent dislocations. A commonly used scoring system for management is described by the Patellar Instability Severity (PIS) Score. Long-term studies show that surgical treatments reduce the risk of redislocation and achieve favorable clinical outcomes. Individualized and risk factor-tailored surgical interventions like trochleoplasties can be utilized to prevent dislocations and restore knee function. Tailored therapy is important to prevent recurrent dislocation and reduce long-term damage such as patellofemoral arthritis.
