<p>With a&#xa0;prevalence of 8–13%, polycystic ovary syndrome (PCOS) is one of the most common endocrine disorders affecting women of childbearing age. Symptoms can include acne, hirsutism, menstrual irregularities, hair loss and metabolic disorders. Standardised diagnostics are essential, as untreated cases are associated with an increased risk of cardiovascular disease, endometrial cancer, and anxiety and depression. The new German S2k guideline on PCOS diagnostics, due to be published in 2025, represents a&#xa0;significant step forward. It enables a&#xa0;structured, interdisciplinary and evidence-based evaluation of this complex clinical presentation. Applying the modified Rotterdam criteria correctly, alongside new tools such as the anti-Müllerian hormone (AMH) value and differentiated phenotyping, enables individualised care. The central goal is to avoid both overdiagnosis and undertreatment, while initiating preventive measures for metabolic, cardiovascular and psychological risks at an early stage.</p>
