Pharmakotherapie des PCOS aus internistischer Sicht

Pharmacotherapy of polycystic ovary syndrome (PCOS) as part of a multimodal treatment concept is challenging due to off-label use and other PCOS-specific considerations. The choice of medication—ranging from oral contraceptives, metformin, and antiandrogens to treatments addressing sequelae and comorbidities—is taken in shared decision-making with the affected woman, balancing substance-specific benefits and side effects. The determination of target goals takes the severity of symptoms, psychological distress, and the preferences of the woman with PCOS into account but also considers the long-term risks of PCOS sequelae and comorbidities such as obesity, diabetes, atherosclerotic cardiovascular disease, and metabolic dysfunction-associated liver disease. In a holistic approach, the treatment concept tailored in interprofessional collaboration should equally address the gynecological and internal medicine facets of PCOS.
