In this review, the importance of individualized contraception advice is presented. In addition to an overview of short- and long-acting reversible contraception methods, an algorithm for principled decision-making is presented. The most important factors which play an important role from the medical perspective are explained. Special aspects of advice in specific phases of life—adolescence and perimenopause—are emphasized. The patientsʼ wishes must be taken into account to achieve good compliance especially when using short-acting contraceptives. If risk factors are present, medical aspects are important for selection of the method. These can range from potential additional benefits for some diseases to existing contraindications. Selected situations, like contraception in obesity, presence of venous thromboembolism (VTE) risk factors or postpartum contraception, are discussed in more detail. Based on guideline recommendations, practical tips for contraception advice in daily practice are given. This requires gynecological endocrinological expertise.
