<p>The preterm birth rate in Germany has remained consistently high for years and is a&#xa0;major contributor to perinatal mortality. Numerous risk factors for preterm birth are now known. The goal is to identify risk factors during the preconception counselling before pregnancy. Ideally, targeted interventions lead to risk minimization before pregnancy begins. This is easier when risks are clearly identified; however, a&#xa0;multifactorial approach is often necessary. Effective intervention options include optimizing chronic maternal diseases before pregnancy, providing information about the timely use of acetylsalicylic acid (ASS) in patients with a&#xa0;high risk of preeclampsia, abstinence from nicotine, alcohol and drugs, optimizing body weight, considering the optimal pregnancy spacing before subsequent pregnancies, reducing iatrogenic multiple pregnancies, considering surgical interventions and future pregnancies, physical training and vaccinations. Despite current knowledge, the ability to influence preterm birth is unfortunately limited. Further research is needed, particularly regarding bacterial causes of preterm birth and their prevention.</p>
