With a prevalence of 6–8%, pre-eclampsia is one of the most common and, in some cases, most serious complications of pregnancy with potentially life-threatening consequences for both mother and child. Because there is still no causal treatment, choosing the optimal time for delivery is of key importance. Here, the potential maternal complications of expectant management must be weighed against increased neonatal morbidity and mortality associated with iatrogenic premature birth. The decision regarding the mode of delivery depends on the severity of symptoms and gestational age. Particularly in cases of milder symptoms from 34 + 0 weeks of gestation onwards, individually tailored delivery planning with the informed consent of the pregnant woman is necessary, taking into account not only maternal complications and long-term consequences but also the higher risk of neonatal respiratory distress in late preterms. In this article, current findings on the mode and timing of delivery and the corresponding recommendations in pre-eclampsia are summarized.
