Minimal-invasive Operationen für Endometriumkarzinompatientinnen
摘要
Continuous improvements in laparoscopic techniques and electrosurgery as well as modern visualizations have made minimally invasive surgery (MIS) an attractive option for patients with early endometrial cancer and comorbidities. The advantages of MIS with smaller incisions, lower pain medication requirements, shorter hospital stays, and lower rates of incisional infections compared to conventional laparotomy are particularly important for patients with endometrial cancer, who often suffer from obesity and other comorbidities. The oncological safety of MIS in patients with early endometrial cancer is still the subject of clinical studies. In particular, the use of a uterine manipulator is not uncritical in early endometrial cancer and must be investigated further. Further progress can be expected in the future, particularly in the field of robot-assisted surgery. The extent of lymphadenectomy must be adapted to the oncological risk profile in order to avoid over- and undertreatment and should also be weighed up against the expected morbidity.
