<p>Proximal femoral fractures are frequent injuries that typically require surgical treatment. A&#xa0;rare complication is iatrogenic injury to the sciatic nerve, often caused by cerclages or suboptimally reduced fracture fragments. Our retrospective study of 224 cases in 39 months revealed an incidence of 1.8% for postoperative sensory motor deficits. The treatment includes revision surgery, such as neurolysis or improved reduction of fracture fragments. Despite surgical revision, long-term deficits often persist due to slow nerve regeneration. A precise operative technique and early diagnostic work-up can help mitigate the risk.</p>
