<p>Breastfeeding is an evidence-based preventive measure with proven benefits for the short-term and long-term health of both child and mother. National and international recommendations emphasize its role as an effective public health measure. Despite this strong evidence base, relevant deficits in breastfeeding support persist in routine care, attributable to the insufficient and inconsistent integration of breastfeeding-related competencies into the medical education and training system. Lactation medicine is only marginally represented in undergraduate medical curricula, while postgraduate and continuing education options are heterogeneous and lack standardization. This negatively affects counselling quality, clinical decision-making and continuity of care. This article classifies the German National Breastfeeding Promotion Strategy, with particular emphasis on the strategic field basic and advanced training and continued professional development, together with the work of the National Breastfeeding Committee, as the health policy framework for building professional competence. Furthermore, emerging qualification pathways are described using qualification in lactation medicine and breastfeeding counselling for physicians as an example. This qualification is discussed as a&#xa0;structured approach to addressing the existing gap in care. The article concludes by outlining perspectives for the sustainable integration of breastfeeding competence into medical education, postgraduate training and interprofessional care structures.</p>
