摘要

Medical handovers transfer information, responsibility and accountability for patients and are crucial for patient safety. In pediatric emergencies, they carry a very high risk of information loss as factors such as stress and lack of routine increase the likelihood of errors. Standardized structured communication, crew resource management principles and (digital) tools are crucial for minimizing risks and ensuring treatment safety for young patients. The literature shows that structured handovers based on memory aids reduce information loss and increase safety for patients and staff. The choice of tool often depends on the setting, medical specialty and local practice. The effectiveness of structured handover depends on standardization, clear framework conditions, sensible timing and continuous training. Visualization, checklists, whiteboards and interprofessional simulations support its application. A handover scheme can only be permanently and effectively implemented through consistent training, adaptation to the area of application and a lived safety culture.
