Background

Hydatids of Morgagni are small cystic lesions, which arise embryologically as remnants of the mesonephric duct. While mostly asymptomatic they can lead to acute abdominal pain due to torsion.

Case presentation

This article reports the case of an 11-year-old girl presenting with severe lower abdominal pain, and a hypoechoic cystic structure in the area of the right ovary on ultrasound. Laparoscopically a torsioned hemorrhagic hydatid with a torsion of the right fallopian tube was identified, which was successfully managed with cystectomy and the postoperative recovery was uneventful. This publication summarizes cases reported in the literature.

Discussion

Hydatid torsion is among the rarest causes of acute abdominal pain in girls of childhood age. In some cases it is accompanied by torsion of the fallopian tube. The differential diagnostics with an ovarian torsion are difficult, therefore laparoscopy is always warranted in such cases.
