Epileptic seizures are among the most common emergencies in childhood and adolescence with a wide variety of causes. In the present case report, a 20-month-old infant developed status epilepticus with fever and sinus tachycardia, compatible with complex febrile seizure but during physical examination by the emergency physician, bilateral ocular mydriasis was noted. Ultimately, the cause of the status epilepticus, fever, and tachycardia was an accidental amphetamine intoxication. During the following 50 hours in pediatric intensive care, profound agitation and arterial hypertension were also noted as further symptoms of intoxication. The child was treated symptomatically (antiseizure medication, invasive ventilation, stimulation shielding and sedatives, antipyretics, volume substitution, and intensive care monitoring) and was discharged home in a neurologically unimpaired condition after 10 days.
