Background <p>Accessory breast tissue is a&#xa0;residual glandular mammary tissue that persists due to incomplete regression during embryonic development. It typically occurs along the embryonic milk line and is influenced by hormonal changes. While most cases are reported in the axilla, other regions along the milk line, excluding the pectoral and vulvar areas, can also be affected. To our knowledge, this is the first documented case of accessory breast tissue within the pelvic cavity extending to the buttock.</p> Case presentation <p>A&#xa0;35-year-old Iranian woman presented with a&#xa0;left-sided pelvic cavity mass, buttock asymmetry, and induration but remained asymptomatic. Initial imaging and biopsy suggested lipoma or liposarcoma, leading to complete surgical excision. Histopathological analysis confirmed ectopic breast tissue. The patient had no family history of breast abnormalities. She recovered fully and remained disease free over 4&#xa0;years of follow-up.</p> Conclusion <p>Accessory breast tissue is typically located along the embryonic milk line, with the pelvic cavity and buttock being exceptionally rare sites of occurrence. This case highlights the importance of considering accessory breast tissue in the differential diagnosis of pelvic masses, particularly when hormonal responsiveness is observed.</p>

Background

Accessory breast tissue is a residual glandular mammary tissue that persists due to incomplete regression during embryonic development. It typically occurs along the embryonic milk line and is influenced by hormonal changes. While most cases are reported in the axilla, other regions along the milk line, excluding the pectoral and vulvar areas, can also be affected. To our knowledge, this is the first documented case of accessory breast tissue within the pelvic cavity extending to the buttock.

Case presentation

A 35-year-old Iranian woman presented with a left-sided pelvic cavity mass, buttock asymmetry, and induration but remained asymptomatic. Initial imaging and biopsy suggested lipoma or liposarcoma, leading to complete surgical excision. Histopathological analysis confirmed ectopic breast tissue.
