<p>Tailgut cysts are a&#xa0;residual of the embryonic development and are a&#xa0;rare primarily benign finding in the retrorectal space but they have the potential of becoming malignant. The majority of patients are asymptomatic but symptoms may occur due to the space-occupying effect. Based on a&#xa0;case report the transanal transrectal endoscopic resection with the transanal endoscopic microsurgery (TEM) instruments is described and the necessary diagnostics and possible surgical strategies are discussed.</p>
