Chronischer Sinus nach Rektumexstirpation

A chronic pelvic sinus after abdominoperineal resection is a frequently occurring problem in colorectal surgery, especially in patients with Crohn’s disease who already suffer from an anal fistula and in previously irradiated patients with rectal cancer. Exact data on the incidence are lacking as well as an exact definition. The treatment is often unsuccessful. The most effective measures are presumably various muscle flaps, such as a gracilis muscle flap, which is however invasive. The article describes the approach of the author, whereby the first step is the insertion of an endoluminal vacuum treatment which is removed after changing 3–4 times followed by a primary wound closure.
